*Śīrṣaśūlahṛt*

*Bhrūśaṅkhakarṇaśūlāni tathā'rdhaśiraso rujam*

*Sūryāvartaṁ śaṅkhakaṁ ca dantapātaṁ ca tadrujam*

*Naktāndhyaṁ paṭalaṁ śukraṁ cakṣuḥpīḍāṁ vyapohati*

Headache, as well as pain = ache of the brows, temples, and ears; pain of Ardhavabhedaka (migraine/hemicrania), pain of Suryavarta, Shankhaka, pain arising from tooth loss, and useful in diverse eye diseases.

The organ affinity (gamitva) of Pathyadi is the head (shira); it is especially useful in Vata-predominant disorders.

Because it contains Triphala, it is also useful (to a certain extent) in ocular disorders.

Guda (jaggery) is a sugarcane derivative, which is also indicated with Haritaki in Pratishyaya (coryza/rhinitis)...

Also, because Guda is Abhishyandi (secretory/channel-blocking), it is an Utkleshaka (toxin-agitating/mobilizing) substance; due to this reason, along with Vata, it is capable of detaching and expelling Pitta and Kapha—these manifest, Prithvi-Jala-predominant obstructive factors—from the micro-channels, from deep-seated/lodged spaces... from sinuses... from ethereal/cavitary spaces.

Together with this, because it contains Triphala, it is useful in the form of dryness for the absorption of Kleda (excess moisture), and in the form of Virechana (purgation) as Anulomana (downward movement) or Shodhana (purification).

Triphala is included among the drugs of intense smoking (Tikshna Dhumapana), which is the chief line of treatment for Vata-Kapha disorders located above the clavicle (Jatrurdhvagata); therefore, it is an Uttamanga Shodhana (purifier of the head/upper organs).

Bhunimba, that is Kiratatikta, is the main ingredient of Mahasudarshana, which is useful as an antipyretic, meaning digestion of Ama (undigested toxic waste)... It is inherently laxative, meaning mildly purgative (*jvarānte recanaṁ dadyāt*).

Haridra is the foremost drug for Meha (urinary disorders/diabetes); due to this reason, it is a destroyer of Kleda. The properties of Daruharidra are similar to Haridra; therefore, it is useful as an Uttamanga Shodhana.

Nimba is included in Panchatikta, and is a destroyer of Krimi (parasites/microbes) and Kushtha (skin diseases). Moreover, Nimba is the primary and sole levigating (bhavana) substance of Arogyavardhini; therefore, it is a destroyer of Kleda-predominant (Kapha-Pitta) disorders like Kushtha.

Guduchi is a destroyer of Vatarakta, which means again it is a destroyer of Kleda, and it is a destroyer of Shonita-vibandha, meaning it is an obstruction remover. Furthermore, by being a Shamana (pacifier), Rasayana (rejuvenator), and Medhya (nootropic), it is an Indriya Prasadaka (clarifier of the sensory organs).

In this manner, by Guda = oleation and mobilization (Snehana-Utkleshana); by Triphala = purgative and purificatory processes (Virechana-Shodhana); and by Nimba, Kiratatikta, Haridra, Guduchi = pacifying processes through Kleda absorption, drying, and clarification (Rukshana-Prasadana)... Pathyadi is capable of pacifying especially Vata-born conditions, as well as secondarily pacifying Pitta and Kapha that provoke Vata through obstruction.

Along with the gross, wide, or umbrella-like organ that is the head (shira),

The meaning of head includes intra-cranial spaces = those cavities, and peri-cranial surrounding spaces = sinuses including maxillary, frontal, sphenoid, ethmoid, and mastoid cells; and all other spaces occurring in this region including the eyeballs, Eustachian tube connecting the ear and throat, nose, maxillary sinus, nasolacrimal duct, nasal meatus, oral cavity, throat, pharynx, larynx—all these ethereal components, spaces, sinuses, vacuoles, tubes in the supraclavicular region... as well as the spaces within the external, middle, and internal ear; which, in the form of the cochlea and vestibule, govern balance and spatial orientation. In all pathologies occurring in all these organs—principally Vata-born, and secondarily Pitta-Kapha-born—Pathyadi works as a *balanced formulation*, through its affinity (gamitva), its dosha-pacifying nature, its obstruction-clearing nature, as a purificatory process... and in the form of pacification, clarification, and alleviation of irritation...

In which...

Guda performs the work of oleation & mobilization;

Triphala performs the work of drying, purification, and downward regulation; and

Nimba, Kirata, Guduchi, Haridra provide universally beneficial action in the form of pacification and clarification.

Therefore, in head diseases presenting with any type of pain... whether it is

Modern-day migraine, or

Trigeminal neuralgia,

Heaviness of the head caused by sinusitis, or

Temple pain (Shankhashula) associated with symptoms or relief of Amlapitta (hyperacidity) and vomiting, or

Earache / heaviness in the ear / ear discharge / tinnitus (Karnanada) caused by external or internal factors, or

Middle ear inflammation/infection, or

Inner ear hearing loss, or imbalance / dizziness / vertigo type of disorder, or

Parietal, temporal, occipital, frontal headache that is

Arising from heat/sun exposure, anger, grief, or elevated blood pressure; or

Headache arising from Vata, Kapha obstruction, Pitta inflammation, or sinusitis; or

Pathologies born of doshas in the eyeball; or

Disorders predominant in or born of Vata, Vata-Kapha, Vata-Pitta, or Pitta-Kapha in the nose (Pratishyaya, sinusitis, sneezing, nasal blockage, watery nasal discharge, itching in the nose), throat (tonsils, pharyngitis), oral cavity (stomatitis, toothache, gingivitis)—

Through one mode of action or another, Pathyadi is definitively relieving...

In which Triphala is functional, useful, and effective in the form of purification, drying, and ocular benefit.

Guda, being sweet, heavy, and unctuous, is a Vata pacifier; and due to its Abhishyandi nature, it is a dosha-mobilizing agent (Utkleshaka), meaning it expels doshas lodged within the channels. And

Nimba, Kiratatikta, Haridra, Guduchi—these remaining four substances—act through their innate potency, disease-curing capacity, and richness of properties as Kleda absorbers, dosha pacifiers, clarifiers, & destroyers of tissue vitiation (dhatu-dushti).

For this reason, not merely the head or eyes, but across the entire supraclavicular region—head, cranium, temples, ears, eyes, nose, teeth, mouth, tongue, throat, pharynx, larynx, sinuses, mastoid, Eustachian tube, cochlea, vestibule, middle ear, external & internal ear, as well as intracranial and pericranial spaces—in all pathologies occurring in all these sites, mainly Vata-predominant and Pitta-Kapha-born, Pathyadi is definitively effective.

**Dosage**

If the intensity of disorders or pain is severe: 6 tablets, 3 to 4 times a day, after meals or at night before sleeping during sleep-time = bedtime...

If the intensity of disorder or pain is moderate: 4 tablets &

If the intensity of disorder or pain is mild: 2 tablets 👆🏼

Administering its dosage in this manner is useful.

**Anupana (Vehicle / Adjuvant)**

Guda cannot be directly included into the tablet; therefore...

Guda / Gudodaka = water dissolved with jaggery,

Warm water, or

Mung bean soup (Mudga Yusha), or

Milk decoction prepared with fresh ginger or garlic (Kshirapaka), or

Ghee & warm water.

Administering its vehicle according to the disease, season, dosha, age, and habitat can be useful.

**Duration**

For acute pain/colic, relief is possible within half an hour (30 minutes) to 3 hours.

And for long-standing, chronic diseases, continuous use of this formulation for 2 weeks to 6 weeks = 14 to 42 days may be necessary.

**Other Formulations / Combinations**

1. If there is an excess of pain: Duralabhadi Vedanashamaka or Gridhrasinashaka.

2. If the pathogenesis or cause is due to tissue depletion (dhatu-kshaya): Vidaryadi / Yashti Gokshura Kalpa / Kushmanda Kalpa / Shaliparni Kalpa.

3. If agitation, mental anguish, emotional, psychological, or intellectual stress is the cause: Yashti, Sariva & ghee, milk Nasya, Tarpana, eye Pichu.

4. If Agni, Pitta, Rakta, heat, or a history of viral infection, typhoid, malaria, dengue, chikungunya is the cause or history: Vasa-Guduchyadi.

5. If symptoms of Amlapitta (hyperacidity) are concurrent and rapid relief occurs through vomiting: Bhunimbadi.

6. If symptoms such as Pratishyaya, Pinasa, sinusitis, rhinitis are present: Uttamanga Shodhana.

7. If there is constipation (Malavarodha): Any routine mild laxative (Nitya Mridu Virechana) alongside.

8. Since Rakta (blood) is the dushya (vitiated tissue) in head diseases, wherever possible & necessary, performing bloodletting (Raktamokshana) is useful.
