A. 1.
**“Tikta-rasa-containing Ksheera-Ghrita Basti”** is the treatment for **Asthi-kshaya** (bone depletion), as stated in **Ashtanga Hridaya Sutrasthana Chapter 11 & Charaka Sutrasthana Chapter 28**.

Therefore, **Panchatikta** — in the form of **Saptadha Balaadhana Tablets or Ksheerapaka Sharkara Kalpa Granules** — for acting upon the bones,
is useful when given **before all three meals = before breakfast / before lunch / before dinner = during Apana period = Pragbhakta period**, along with **milk and ghee**.

Because **Asthidhara Kala and Purishadhara Kala are one and the same**, and are situated in the **Pakvashaya**,
and

**“Tikta-rasa-siddha Ksheera-Ghrita”** has been instructed to be given as **Basti**, and this Basti acts in the **Pakvashaya**. The controller of the Pakvashaya is **Apana Vata**, and the therapeutic time of Apana Vata is **Pragbhakta**, i.e. before food.

Although **“Tikta Ksheera-Ghrita Basti”** is the instruction of the Shastra, making Ksheerapaka or Siddha Ghrita requires time, and drinking such a highly bitter medicine in milk / ghee is not convenient for the patient!
6. **Alternative method of preparation**
Therefore, the Ksheerapaka prepared by mixing **5 grams of Panchatikta Churna in 80 ml milk** — the amount of extract that enters the milk is calculated by volume through pharmaceutical calculation, and the same amount of Panchatikta extract is incorporated into **one flat spoon of sugar**. In this manner, **Panchatikta Granules and Saptadha Balaadhana Tablets** have been prepared! ✅️
If these granules are given **before food, along with milk and ghee**, they provide the therapeutic result of **Tikta Ksheerapaka**.
The quantity of these granules is **one flat spoon three times, before meals** ... or **2 to 6 tablets before meals**.
7. **Dose & Duration**
If the **severity / chronicity of the disease & age / strength of the patient** are not favourable to that extent, or if the **pain / colic is severe / strong / unbearable**, or if the nuisance value of the complications of the disease is greater, then in such a situation the same granules may be given as **two or three flat spoons before meals = 3 or 6 or even 9 grams** ...
Or alternatively, **Panchatikta Saptadha Balaadhana Tablets** should be given in a dose of **2 / 4 / 6**, according to the intensity of pain, chronicity of the disease, and the age, strength, etc. of the patient.

Now, in patients who have a problem of **diabetes ๐ค⁉️**, even here, because these granules are given **“before food”**, sugar / carbohydrate / glucose does not increase, because when they are taken **before food**, the patient naturally eats four-five fewer morsels / boluses of the food that is subsequently taken; because the meal itself begins after eating two spoonfuls of sugar / granules.
Or instead of the granules, it is appropriate to give **Panchatikta Saptadha Balaadhana Tablets**.
9. **Mode of Action — without giving even a single Basti**
In this manner, because **Panchatikta Granules or Tablets** are used **before food**, they act upon the **Apana region**, i.e. during the **Pragbhakta period**, and act upon the **Pakvashaya**, which is the principal location of Vata ...
Therefore, definitely, similar to the Basti mentioned in the original reference, beneficial and expected results are obtained rapidly and within a short period ... This is not merely a theoretical, philosophical or hypothetical statement ... rather, this formulation incorporates the essence of **3000+ cases from 28 years of clinical experience!!!**
... and that too, **“without giving Tikta Ksheera-Ghrita Basti even once to a single patient, ever.”**

At the same time, there is another fact which is more serious: **Tikta Rasa is the only Rasa that creates Akasha Mahabhuta**. Therefore, even in a condition such as **Asthi-kshaya**, which is an increase of Vata, **Tikta Rasa**, which itself increases Vata, is specifically prescribed in the treatment of Asthi-kshaya. This is clearly written in **Arunadatta's commentary**.
So, if **Tikta Rasa**, which is composed of **Akasha and Vayu Mahabhutas**, produces a therapeutic result for **roughness and porosity in the bones**, then similarly ...
wherever there is an obstruction, especially in the **cardiac vessels / cardiac arteries**, where there is obstruction in the form of **plaque or thrombus** in the three major vessels, **Panchatikta Tablets prepared by the Saptadha Balaadhana method or Panchatikta Granules** also provide beneficial results!
**Except in extremely serious / emergency conditions!!!**
2. **Obstruction Removal = Blockage Clearance**
Similarly, wherever there is an **obstruction produced by Prithvi-Jala predominant, Sthula-Guna** factors, there the **Vayu-Akasha predominant, Sukshma-Guna predominant, Vivaraแนa i.e. expansion of the Srotas = obstruction removal = blockage clearing** — from this perspective, Panchatikta Saptadha Balaadhana Tablets & Panchatikta Granules are useful.
Therefore, in **AVN** also, the use of **Panchatikta Tablets or Granules** provides beneficial results.
3. **Treatment Duration & Non-Recurrence in AVN**
However, for this, patience and waiting are required! Generally, according to the **grade of AVN**, treatment is required for **100 to 1000 days** ... but thereafter, the results obtained and the remission achieved are **long-lasting and permanent** ... the patient benefits from this treatment in the form of **non-recurrence / Apunarbhava**!
Patients with AVN who were treated and cured **20/22 years ago** are still well today, free from pain, walking on their feet, running and jumping, and carrying out their daily activities efficiently.

### D. 1. Panchatikta & Liver Heat and Pitta-Rakta-Agni +++
Because Panchatikta itself contains **Vasa, Guduchi and Nimba**, just as **Vasaguduchyadi** is highly useful in conditions of **“Liver Heat” / Pitta-Rakta-Agni heat +++**, similarly **Panchatikta** is useful in such conditions.

Also, because **Vasa, Guduchi and Patola** are present in Panchatikta, just as **Bhunimbadi** is useful in the pathway of various disorders related to **Amlapitta, Sheetapitta**, and **Prithvi-Jala predominant, Drava-Kleda predominant disorders**, and allergy-related disorders ... it is useful in disorders caused by **Kleda moving from the Koshtha towards the Shakha** ...
similarly, **Panchatikta**, in the absence of Bhunimbadi, is useful as a **representative, secondary and subordinate** option.

To destroy **“Prithvi-Jala predominant, Sthula-Guna-containing obstruction”**, **“Akasha-Vayu predominant, Sukshma-Guna-containing Tikta Rasa”** is used ...

**Ksheerapaka** is a very powerful and highly effective pharmaceutical preparation.
2. **Special Utility & Duration & Other Supporting Medicines**
Through **Tikta Ksheerapaka**, many patients with **Asthi-kshaya**, such as **L5-S1 degeneration, slipped disc, disc bulge, AVN, sciatica, spondylosis, knee degeneration, ACL, osteoarthritis**, etc., have experienced benefit.
This benefit is obtained **without giving Basti**, merely within **one Mandala, i.e. 42 days, i.e. 6 weeks**.
If **Panchatikta Tablets / Granules**, ... only plain **Guduchi / Yashti-Guduchi / Yashti-Laksha**, in the form of tablets or granules, are used along with **milk and ghee, before food, i.e. Pragbhakta, i.e. during Apana period**, excellent beneficial results are obtained rapidly within a short period.

Because the **Tikta Ksheera Basti**, which is instructed in the Shastra, acts in the **Pakvashaya itself** ... the master of the Pakvashaya is **Apana Vayu**, and the therapeutic time of Apana is **Pragbhakta, i.e. before food**.
Therefore, when **Panchatikta + Yashti-Laksha / Guduchi / Yashti-Guduchi** are given in the form of **Saptadha Balaadhana Tablets or Granules** during **Pragbhakta / before-food period**, they produce such excellent results in the form of beneficial therapeutic effects causing **Apunarbhava / non-recurrence**.
This is the experience of **3000+ patients over 28 years of clinical practice**.

Such rapid results can be obtained from **Yashti-Guduchi Ksheerapaka or Panchatikta Ksheerapaka**, according to the experience of the last **28 years**, and these results are of the nature of **no recurrence / Apunarbhava**.
That means, over the last 28 years, there is experience of **more than 3000 such cases**, among which many surgical operations have been avoided, and patients who were cured **20/22 years ago** are still equally well today and continue to refer other new patients even today.

Patients who are lying on the bed, whose **SLR is less than 10° or near zero 0°**, whose **Apana-region activities (defecation and urination)** are being carried out on the bed itself with the assistance of other people, also come walking on their own feet after **42 days or at most 84 days (6 to 12 weeks)**, climb up and down one or two floors, and begin carrying out all their daily activities comfortably as before.

7. **Non-healing & Delayed Fractures**
Even in **Bhagna / fractures**, **Panchatikta Tablets or Granules**, along with **Yashti-Laksha Tablets or Lashuna Granules or only Guduchi Granules or Yashti-Guduchi Granules**, are highly effective & prove beneficial according to the condition, in **non-healing fractures or degenerative conditions of the spine, knee, hip joint, cervical and lumbar regions**.

However, as stated earlier, while preparing such **Ksheerapaka**, it is an artistic, technical and time-consuming process to retain the appropriate quantity of milk, stop the heating at the exact time, allow all the water to evaporate, and leave only the milk containing the extract of the medicinal drug completely remaining.

Therefore, so that the benefits of **Ksheerapaka**, this nectar-like pharmaceutical preparation, may be obtained, while eliminating the difficulties associated with it — such as its difficult preparation, **stability / shelf life (very short expiry time), palatability due to bitterness, storage facility and transportability** — this **Ksheerapaka Sharkara Granules & Saptadha Balaadhana Tablet** is being presented with a new idea.
In the proportion in which the medicinal extract enters the Ksheerapaka made with **80 ml milk**, if the **Sharkara Kalpa** of the drug whose Ksheerapaka is to be made is added to the same amount of plain / ordinary milk in an appropriate proportionate quantity, in grams or volume, then an equal amount of the drug extract can be made available in the milk.
Alternatively, **6 Saptadha Balaadhana Tablets of 250 mg each**, dissolved in **80 ml warm water or warm milk**, can provide the same medicinal effect.

This is a **pharmaceutical calculation**, in which, by determining how much medicinal extract is obtained from Ksheerapaka through both options, **1:16:16 or 1:8:32**, and calculating the appropriate proportion of **drug : water : sugar**, the quantity is determined.
Thus, if **5 grams or 6.25 grams (½ Karsha)** of the medicinal preparation in the form of **Sharkara Kalpa Granules or Cube** is added to **80 ml or 100 ml (2 Pala) milk**, the same Ksheerapaka is obtained, along with the medicinal potency.

In **Asthi-kshaya-related disorders**, the three preparations — **Panchatikta Saptadha Balaadhana Tablet & Kalpa = Ksheerapaka Sharkara Granules, Guduchi Kalpa and Guduchi-Yashti Kalpa** — act as **Tikta Ksheerapaka**, and are useful along with **Yashti-Laksha Tablet**.
Tikta Ksheerapaka is mentioned in **Asthi-kshaya**. Along with this, if we look at the **Bhagna Adhikara**, the reference comes in the form of **“Madhura Aushadha-siddham Laksha-yutam”**.
Therefore, **Guduchi has been included along with Yashti** here, so that both **Madhura and Tikta Rasas** can be obtained in the form of Ksheerapaka.
14. **Special Indications & Utility of Panchatikta + Yashti-Laksha + Yashti-Guduchi**
Going further, **Panchatikta Saptadha Balaadhana Tablets or Sharkara Kalpa = Granules** are more useful.
In **Manya, Kati, Janu Shoola**, and in the **Asthi-kshayatmaka Samprapti** of **ACL, AVN, osteoarthritis, sciatica, L5-S1 degeneration, lumbar and/or cervical spondylosis**, etc., among these three — **Panchatikta + only plain Guduchi + Yashti-Guduchi** — any one **Ksheerapaka Granule = Sharkara Kalpa**, along with **Yashti-Laksha**, provides the expected result.
Results similar to Panchatikta are obtained from **Lashuna Granules**, which are **Bhagna-Sandhanakara** and, as stated in **“Prabhaรฑjanaแน Jayati Lashunaแธฅ”**, are the supreme medicine for Vata.

Panchatikta is useful not only in **bone fractures**, but also in **skin diseases**!
If this **Sharkara Kalpa Granule or Saptadha Balaadhana Tablet**, an innovative pharmaceutical preparation, is given with **milk or warm water**, then any medicine can be made palatable for a child.
Since it is a **Sharkara Kalpa**, it can be given with milk or water, and similarly this Panchatikta Sharkara Kalpa medicine can also be given with **tea or coffee**, or the Panchatikta Granules can be spread over a roti, or the Panchatikta Tablets can be powdered and spread over a roti, with ghee applied and made into a roll.

17. **Dose, Anupana & Duration for Skin Diseases**
Especially, **Panchatikta Tablets**, prepared by the **Saptadha Balaadhana method**, having **sevenfold potency**, are highly beneficial in **Kuแนฃแนญha, i.e. skin diseases**, when given **after food, three times a day, i.e. after breakfast / after lunch / after dinner**, with **warm water or Mudga-Yusha** (or, if the Kuแนฃแนญha is dry and rough, with good-quality pure homemade desi ghee).
According to the extent and chronicity of the skin disease, excellent remission in **skin disease = Kuแนฃแนญha** is obtained within **6 weeks = 42 days or 12 weeks = 90 days**.

In any case, whatever therapeutic action **Arogyavardhini** has is due to the **Bhavana with Nimba leaves** and the **purgative action of Triphala and Katuki**, and **Nimba is already included in Panchatikta!**
19. **Dose & Duration**
According to the extent of **Kuแนฃแนญha / skin disease** in the patient and according to the severity of symptoms, it is appropriate to use **Panchatikta Saptadha Balaadhana Tablets — 6 tablets three times / or 4 tablets three times / or 2 tablets three times**.
How much dose should be given should be decided by the **Vaidya after assessing the patient**.
20. **Benefits from External Application — External Application Usage**
Along with this, application of **Panchatikta itself or TNKMJ Churna**, mixed with **Masoor Dal flour**, over the entire body, and after allowing that application to dry, taking a bath with warm water, and thereafter wiping the skin with a cotton cloth and, after it becomes dry, applying **Eladi Ker Taila or Shatadhauta Ghrita** ... or, if there is bleeding or any other discharge, applying **Jatyadi Ghrita / Taila**, is beneficial as an external treatment, to some extent.
21. **External Don'ts**
Throughout this treatment period (= **6 months**), **soap, shampoo, face wash and any such chemical cosmetics that produce foam** should not be used on the body.

And in the diet, **sour taste, salty taste, Maida-based products, green leafy vegetables (Patra Shaka), underground-growing foods & foods cooked in a cooker** should be completely avoided, for **twice the number of days for which the treatment is continued**!
22. **Mahabhuta Consideration**
**Tikta Rasa** is the only Rasa containing **Akasha Mahabhuta**, and Akasha Mahabhuta is **Sattva-dominant**; Sattva is light and illuminating.
Along with this, because it is **Medhya**, it is useful in **Tamo-guna predominant**, i.e. **Prithvi-Jala predominant**, i.e. **Kleda-predominant disorders**.

The two best drugs having **Tikta Rasa**, namely **Guduchi and Patola**, are both included in Panchatikta.
Among these, **Guduchi is also a Medhya Rasayana**.

In **Sharngadhara Samhita**, Panchatikta is mentioned in the context of **Vishama Jvara**, and in the classical treatment principle of fever, **Tikta Rasa** is clearly mentioned independently as the principal treatment.

**Vasa** — of Raktapitta,
**Guduchi** — of Vatarakta, and
**Kantakari** — of Kasa,
are three **Agreya Dravyas**, and all three of these best / foremost drugs are included in Panchatikta.

And **Patola** is a principal **Shodhana Dravya for Amlapitta & Sheetapitta** according to the treatment described in **Yogaratnakara**!
