Management of DM type 2 and obesity.
Diabetes type 2 is not prameha because diagnosis of Prameha as per Ayurveda Samhita depends on prabhut and Avila mutra means mutra quantity is excessive and appearance of mutra is turbid but the quantity being excessive is very subjective obscure and vague statement because in Ayurveda Samhita, there is no reference available of normal quantity of mutra in a day, neither normal frequency of mutra pravritti in a day for a Swastha person. so on which basis, one can decide whether this excessive or not. same applies about turbid avila mutra.
Moreover nowadays, neither diabetes is diagnosed on the basis of symptoms related to urine! Gone those days when diabetes was diagnosed on the basis of polypepsia polyphagia and polyuria. nowadays confirm diagnosis of diabetes depends on blood sugar estimation fasting and PP and more correctly nowadays it is confirmed on the basis of HBA1C. so it is not possible for any Ayurveda doctor in India to diagnose on the basis of comparative excessive quantity as it cannot be decided because the basic normal value or quantity or frequency of mutra in non diseased nirogi Swastha person is not mentioned anywhere in literature.
moreover treatment given for Prameha never manages blood sugar.
Lastly, samprapti of Prameha is to expel out kleda along with mutra via / through Basti ... but diabetes is not confirmed or diagnosed on this mutra urine basis.
Reliable authority corporate pathology Laboratories like Metropolis have already displayed a printed notice in all there sample collection centres that , "they have stopped urine glucose test, as it has no relevance for treatment of diabetes"
Diagnosis of Prameha on the basis of mutra quantity and its any equation with diabetes is impossible and unnecessary and useless as DMtype 2 is now diagnosed As Sugar in blood which can be easily understood in terms of Ayurveda as Kleda in Rasa and Rakta!
for my personal understanding I do not consider Rasa as a separate dhaatu, it is neither demonstrable , not dispensable what it is called as Rasa dhaatu. therefore it is better to say that diabetes is raktagatha Kleda. Kleda is mixture of Prithvi and Jala, dominated by jala especially Drava Guna.
for me Diabetes mellitus is not a disease of sugar ... but it is a disease of inability to use the sugar which is consumed by the person or patient.
it is not important or useful to bring down sugar level by giving medicine! then even though you use medicine to lower or decrease blood sugar for lifetime , still next day patient has to take medicine to manage his sugar.
better treatment for diabetes mellitus is to restore the capacity of sugar management system, which is there innate by birth provided to every living body by nature.
sugar management depends on liver pancreas and muscles. so an attempt to correct their capacity to manage use consume the uptaken sugar through daily meals, is the correct direction to treat a diabetes patient.
We MhetreAyurveda claim that from any number of HBA1C, may it be 12 13 or 14 ... it is many times possible to bring the patient at very normal HBA1C level 5.4, within 100 days and after that we may taper out the ongoing modern medicine within next 98 days and after that we will taper out our ongoing Ayurvedic medicine in next 49 days पादेन अपथ्यम् अभ्यस्तं पादपादेन वा त्यजेत् ... so it is in all 250 days treatment to restore the original by birth innate natural god given capacity of patient's body to manage sugar at normal level ... even after completely stopping medicine from Modern medicine and ayurvedic both.

dinner is the culprit !!!
nowadays most of the people have sedentary Lifestyle with almost no exercise after 30 + 40 + age.
generally after youth, or after getting married or after getting into job the lifestyle is to wake upx / get up from the bed , sit on the commode, then sit on dining chair for breakfast , then sit on the two wheeler or four wheeler or public transport to reach to college or to office or to work place , then sit there for next 8 hours, then come back in again two wheeler four wheeler public transport sitting on a given seat , then by reaching home sit on sofa and watch your TV or mobile social media or laptop , then sit on dining chair to have your dinner and then sleep lie down flat on your bed for next 6 to 8 hours ... this is a General routine schedule of almost all the A B C class City citizens who are having sedentary Lifestyle, without any exercise and having 30+ or 40+ age who fall pray to Diabetes mellitus.
now in your growing age from 15 to 25, it is necessary to have full three diets/meals, but after 25, especially after 30 + there is no development in your body, so you really do not need 3 full meals.
breakfast maybe useful for the energy in first half of your daily activities and lunch maybe useful for remaining half day activities as you speak see listen talk Roam walk climb drive ... so in all these activities you may consume the carbohydrates sugar glucose which you have taken into your body through your breakfast and through your lunch ...
but dinner is the culprit !!!
the carbohydrates glucose sugar which is consumed in form of Roti rice fruit milk Bakery ice cream and what not , in the dinner ... remains "unused" , because after that no physical activity is there at all, person just sleeps Lies down flat in his bed for next 6 to 8 hours and he wakes up to take to have to consume a king size breakfast again full of Carbohydrates glucose sugar like poha Upma Idli Dosa or dry fruits or milk ... so the energy or the glucose Carbohydrate sugar or calories which are loaded into the body through the dinner items, "remains unused" for days and weeks and years together ... which eventually converts into fat obesity lipids and one fine day the sugar management system collapses gives up and person gets diagnosed as DM type 2
so management of diabetes mellitus type 2 is not only drug !!!
But Aushadha, Anna & Vihar upayog Sukha vaha is correct treatment.
there is no formula like "drug and disease"! ... it should not be
being a doctor / a Vaidya , we should educate the patient in terms of Anna that is food , Vihar that is exercise and lastly aushadh , the drug !!!
we doctors or vaidyas are "health consultants", we are not "medicine sellers". therefore we should not see hunt search look for "drug and disease formula".
we should cultivate a culture of Holistic treatment which includes,
first : correction in dietary habits,
then : introduction and discipline of necessary and proper exercise and
lastly : the medicine which will restore the capacity of sugar management system which is and was there until the day of diagnosis of diabetes, from birth innate natural given by God, which should work efficiently for lifetime!!!
Diabetes mellitus in first few weeks or months tends to eat up the musculature of the patient, so the patient is facing muscle wasting protein loss, having an appearance with lean and thin arms hands legs calves and a tummy full of fat round and heavy and suspended, this is the picture of diabetic patient , after few weeks months and years of diagnose as DM type 2.
so the first line of treatment, is to restore and assure the avoidance of muscle loss, to build up his muscles, to ensure the proper protein consumption in his daily food.
so breakfast should be full of protein, not artificial proteins, not commercial protein, but food protein homely protein, that is Mudga masura rajamasha means green gram, lentils, cows eye beans and paneer homemade and goat mutton!
there are many misconcepts about red meat , but goat meat is Sharira dhaatu Samaana and AanabhiShyandhi.
Western people or modern medicine advice this to avoid red meat because they consume lamb beef pork, but in India we generally prefer to eat goat mutton, which is lean and thin which is not obese, which is not fatty, unlike lamb beef and pork.
so goat meat though, it is red meat, it is preferable and safe, as a healthy protein source!!!
poultry chicken is "Industrial flesh" which is generally hormone induced, so it is better to avoid
and fish which come from sea are full of kshaara, so better to be avoided
so breakfast should contain good natural homely protein , so there will be Assurance of no muscle wasting and no protein loss anymore.
then in the lunch at Mid Day around 12 noon as far as possible before 1:30 p.m., lunch should have all the ingredients enough only Carbohydrate, ample protein, optimum only fat, sufficient fibre! so one may consume roti fulka bhakri i.e. carbohydrates, long runners Gourds as vegetables for fibre, fat sufficient for Tadka and good quality salad like radish tender
but better to avoid pressure cooker cooked food items , underground strachy sweeet food items, leafy vegetables and pulpy fluidy items as those contain Kleda Prithvi Jala carbohydrate glucose sugar.
Tea should be not more than two cups a day
and ...
as dinner is the culprit !!! ... in the dinner one should eat full stomach up to satiety, no Starving should be there, no malnutrition should be there ... because Starving again causes protein loss muscle wasting, so dinner should be full stomach with full capacity up to satiety ... "but it should not increase energy calories, it should not supply sugar glucose carbohydrate anymore... so what is a good trick, what is a good moral destraction for the body ... to have full stomach meal without uploading unnecessary glucose carbohydrate sugar calories, which are NEVER EVER going to be used, during the night time, as the person will be sleeping laying down flat in his bed for next 6 to 8 hours.
So dinner is the culprit ... therefore dinner should contain bharijit Dhanya = roasted cereals that is laajaa.

this 3 meal diet design is the first important part of treatment out of 3 pillars of diabetes reversal treatment which includes 1.Anna, 2.Vihara and 3.Aushadha
next is Vihara ...
Vihara is proper and sufficient exercise, as all the persons are generally lazy giving excuses for exercise, doing procrastinations and unwilling for regular exercise, saying that having "no time". so it should be mandatory to have a walk of 70 minutes in one go, preferably empty stomach in the morning before 9:00 a.m.
If not possible then before sunset in the evening
if not possible to go for a walk, then climbing down and up the staircase where they live in multi storey building in a flat
if not possible to go down and climb up then just climb up on step and get down and climb up again Step Up Step down only one step for 70 minutes
one may go for work in the lobby or parking or on the terrace or as in the time of Corona all our patients walked in their own house from bedroom to Passage to hall and back to Passage to bedroom !
so there is no alternative or excuse to 70 minutes muscular exercise
those who cannot walk or stand at all due to there knee or low back pain, they should lie down on their back, in the bed and go for air cycling for 100 times forward and backward
and after that they should sit in a chair and go for spinal twist spreading their hands horizontally to the maximum and twisting their spine for 270° clock wise and anticlockwise for 100 times
These two simple exercises are sufficient for all the muscles in the body, below waist & above waist respectively
daytime sleep must be avoided at any case
one may take there habitual sleep or nap in sitting position on a sofa or in a chair keeping their legs horizontally on next stool or chair in front of them
they may lean not more than 120°, but in any case they should not sleep flat in bed or on so far in 180°, even for a minute
and lastly the thing, that you may be waiting for ... is the "drug"
the drug that we use very abundantly with full confidence, more than 8 lakh tablets of this drug are used by MhetreAyurveda and their students and their friends and their fellow Vaidyas in Maharashtra and all over Bharata
the drug is the boss VachaaHaridradi gana from ashtanga Hrudaya sutrasthana 15 ... we use saptadha baladhana tablets of this gana
the dose is 6 tablets 4 times , if hba1c is above 10 and 6 tablet 3 times , if HB A1 C is below 10
the tablets are given preferably after meals, that is bhojanottara
along with VachaaHaridradi, we use trifala Abhaya &/or mustadi for Kleda Nashana.

many things are common about diabetes mellitus type 2 and obesity
here in obesity the over consumption of food especially having excessive glucose carbohydrate sugar similarly over consumption of such pruthvijala Pradhan food along with fat! this unused energy content or unused Prithvi Jala content gets stored in our body as lipid fat medas, at the most relaxed organ that is tummy vapavhana, as we sit in forward bending position for entire lifetime or most of the time in our daily routine
so tummy is the most relaxed part of our body where no tension is ever exerted, therefore fat accumulation = medas sanchiti, is always at tummy initially!!! after the tummy, the tyres get increased after the tyres on both sides, fat accumulates on hips, then thighs, then arms, then neck and then all over the body ... therefore the famous shloka is "sphik stana udara lambanam"
these areas / organs / spaces in our body, are the least stressed , physically no strain no stress is ever exerted on these parts ... the most moving parts wrist hand ankle calf, these ever accumulate fat ... so tension strain on those parts where fat accumulates, should be given during the exercise!!!
Instead of cycling walking running jogging swimming or jerky sports like badminton etc, backward bending side bending twisting exercises are more useful for inch loss fat loss loose weight shedding
that's why yogasana and Pranayam are useless exercises to manage diabetes and obesity
both yoga and Pranayam may improve stamina and pulmonary capacity of the person , but they have no role in reducing sugar or fat at all
so 70 minutes walking or climbing and getting down through stair or Step Up Step down only one step air cycling &/or spinal twist; all these exercises altogether, are useful to use the sugar and the fat by muscles of the patient
so they will not get accumulated again
if we reduce fat and sugar by giving medicines, the day we stop the medicines, fat and sugar will be getting accumulated again and again
So , to get rid of sugar and fat permanently , one should control and educate the food habits , one should make disciplined consistent exercise and the drug which is Agni Vayu Pradhan which is the and tikta and katu that is VachaaHaridraadi gana in case off sugar and Triphala Abhaya that is Sthaulyahara for obesity which is from Ashtanga Hrudaya Sutra 14 , we prepare saptadha baladhan tablets of these drugs.
